Step 1 - We Strive To Learn About You
Functional Medicine is a journey and works best when there is a mutual commitment between you and your practitioner. The more we know about you, the better we can help. Before your initial consultation, complete your intake forms on the patient portal. These forms help us build your overall health profile and help our practitioners understand you and your situation. The intake forms include-
- Medical History
- Family History
- Lifestyle and Social History
- Symptom Questionnaires
We enquire about prior medications, supplements, and lab testing as part of medical history.
Step 2 - Advanced Testing and Analysis
Our practitioners gain deep insight into your health with the help of cutting-edge lab testing. As part of the assessment, our physicians do a comprehensive evaluation of –
- Autoimmune
- Gut/microbiome
- Hormone imbalances
- Inflammation
- Diet
Before your initial consultation, complete a comprehensive blood chemistry analysis with biomarkers for nutrients, blood sugar, thyroid panel, cholesterol, kidney and liver markers, lipids, inflammation, and more as needed.
In addition, specialty testing, such as food sensitivity testing, genetic testing, microbiome testing, or cortisol testing, may be used to investigate biochemical imbalances, nutritional status and detox assessments, and specific aspects of health. These greatly assist the practitioner in analyzing the root cause of your symptoms.
Step 3 - Initial Consultation & Personalized Treatment Plan
Your initial consultation (online or in-person) with your functional medicine clinician will take 60 to 90 minutes. Your physician will discuss your overall health profile and any previous diagnoses, treatments, and medications you currently take. Additional diagnostic testing will help identify the root cause of your condition.
Your functional medicine doctor will create a personalized treatment plan based on your medical and lifestyle history and advanced diagnostic test results. The treatment plan may include-
- Targeted supplement protocol
- Lifestyle and nutrition guidance
- Stress and sleep management
Depending on your subscription plan, your health coach will assist you in implementing your treatment plan and provide continued support on your journey to good health.
Step 4 - Continuous Collaboration - Online or In-Office
Our team collaborates with you to help you achieve optimal health. Subsequent consultations could last 15 to 60 minutes. Your practitioner will evaluate your progress and continue to follow up on the treatment plan designed for you. Adjustments and “fine tuning” may be made to your program as needed. The practitioner may order additional lab tests to track functional imbalances or deficiencies.
To get the most out of your consultation, it helps to be open about any symptoms you are experiencing, medications you are taking, health history, and lifestyle choices. This information helps our practitioners provide you with the best care you need. Please be assured that all information you provide is confidential.
Throughout your healing journey, we strive to support you by offering quiet attention-focused healing of the body, mind and spirit.
Technology Friendly
We strive to use technology to help in your healing journey, including-
- Complete access to all your health information and lab results via the patient portal.
- Secure messaging to safely communicate with your practitioners.
Insurance Coverage
- Freedom of Choice—We are an “Insurance-Free” practice. We believe in empowering you to determine the best treatment options for your medical needs by avoiding the constraints commercial insurance companies place.
- Out-of-network reimbursement—Our physicians are considered “out-of-network.” Reimbursements are dependent on your insurance plan and are covered by your insurance at the out-of-network level. If you have supplemental insurance, it may cover some portion of the charges your primary insurance doesn’t.
- FSA or HSA—Many members pay for their plans using FSA/HSA dollars. You can confirm if this is an option for you by contacting your individual FSA/HSA provider or benefits administrator.
- How We Help You—Our staff will provide you with guidance and an itemized receipt (superbill) for services that includes all the appropriate diagnostic and procedure codes. You can submit this receipt to your insurance company or FSA/HSA plan for reimbursement. For required blood tests or lab work, our practitioners will provide written orders that you can take to the lab of your choice, which are covered by your insurance.
- Medicare—Our functional medicine practitioners have opted out of Medicare. If you are a Medicare beneficiary, you will not be entitled to any reimbursement from Medicare. All costs for services including prescriptions and laboratory tests ordered will be your sole responsibility.
- Payment at Time of Service—You are responsible for all costs for services rendered, and payment is required at the time of service. You can minimize costs by subscribing to one of our Subscription Plans.
