Step 1 - Exploring You

Functional Medicine is a journey and works best when there is a mutual commitment between you and your practitioner. The more we know about you, the better we can help. Prior to your initial consultation, we require you to fill your intake forms on the patient portal. These forms help us build your overall health profile and help our practitioners understand you and your situation. The intake forms include-

  • Medical History
  • Family History
  • Lifestyle and Social History
  • Symptom Questionnaires

As part of medical history, we enquire about prior medications, supplements, and lab testing.

body composition analysis

Step 2 - Advanced Testing and Analysis

Our practitioners gain deep insight into your health with the help of cutting-edge lab testing. As part of the assessment, our physicians do a comprehensive evaluation of –

  • Autoimmune
  • Gut/microbiome
  • Hormone imbalances
  • Inflammation
  • Diet

Prior to your initial consultation, you may be required to do a comprehensive blood chemistry analysis with biomarkers for nutrients, blood sugar, thyroid panel, cholesterol, kidney and liver markers, lipids, inflammation, and more as needed.

In addition, specialty testing such as food sensitivity testing, genetic testing, microbiome or cortisol testing may be used to look into biochemical imbalances, nutritional status and detox assessments and specific aspects of your health. These greatly assist the practitioner in investigating the root cause of your symptoms.

Step 3 - Initial Consultation & Personalized Treatment Plan

Your initial consultation (online or in-person) with your functional medicine practitioner will be between 60 to 90 minutes. During the consultation, your practitioner will discuss your overall health profile as well as any previous diagnoses, treatments and medications you currently take. Any additional diagnostic testing may be ordered to help identify the root cause of your condition.

Your functional medicine practitioner will create a personalized treatment plan using a functional medicine approach. This is based on your medical and lifestyle history and advanced diagnostic test results.

The treatment plan may include-

  • Targeted  supplement protocol
  • Lifestyle and nutrition guidance
  • Stress and sleep management

Depending on your subscription plan, your health coach will assist you in the implementation of your treatment plan and provide continued support on your journey to good health.

Step 4 - Continuous Collaboration - Online or In-Office

Subsequent consultations could be between 15 to 60 minutes. During this time, your practitioner will evaluate your progress and continue to follow up on the treatment plan designed for you. Any adjustments and “fine tuning” may be made to your program as needed. The practitioner may also order additional lab tests to track any functional imbalances or deficiencies.

We are passionate about preventing illness and optimizing your health. Our dedication shows through caring practitioners and staff who take the time to ensure that your concerns are addressed and answer your health questions.

Technology Friendly

We strive to use technology to help in your healing journey. This includes-

Your First Visit

The other time will be used for checking in and out and any lab tests that may be ordered.During the consultation, your practitioner will review all of your medical records and discuss any previous diagnoses, treatments and medications you currently take. All of the information on the intake forms will be discussed in great detail and diagnostic testing may be ordered to help identify the root cause of your condition. Then, the practitioner will develop an individualized treatment plan based on the findings using a functional medicine approach. A significant portion of your visit will consist of patient education. Your practitioner will address and answer all of your health questions.

  • Scan and upload all your records through our secure patient portal
  • Fax them to our office prior to your first visit
  • Mail or drop copies of your records off to our center prior to your first visit. Please call us if you need any help with this. There may be an associated admin fee if we need to scan your records for you (above 5 pages).

Visit Fees

Any recommended supplements and/or specialized tests ordered are additional fees. Many (but not all) of these tests are covered by insurance. (These tests include saliva hormone tests, stool tests for gut function, nutritional tests, food allergy/intolerance tests, based on individual need)
